Only 1.7% of women over 40 are taking hormone therapy. 😩

Only 3.5% of women in their 50's take hormones.

This Mayo Clinic study from 2026 looked at hormone prescriptions through 2023.

We have no problem prescribing other medications to treat many of the downstream effects of hormone loss but we won't prescribe the hormones that potentially fix the underlying problem. ❎

Hopefully, with the recent black box warning removal we'll see improvements.

You've been sold a half truth. 😲

Yes, muscle is the longevity organ. Yes, protein matters.

But sarcopenia, the slow strength-loss that quietly steals women's independence, doesn't start
in the muscle.

It starts in the nerve at the neuromuscular junction. The wiring frays before the muscle ever shows it.

Speaking and traveling are so fun. 🎉

I talked to a group of trail-blazing regenerative physicians in Vegas at @thestemcellconference this weekend.

The topic? How ovarian hormones affect not only female longevity and healthspan but our ability to heal and benefit from stem cell procedures once those hormones are gone.

I've been injecting stem cells since 2014. At that time, it was considered quite "fringe"

Luckily, medicine is changing. And I'm here for it!

What hasn't changed ... me, frantically huddling
in hallways and bathroom stalls, changing clothes on the fly and packing/repacking before making airport runs.

Ah, the glamour! 😂

Hormone therapy and dementia... 🤔

While we don't (yet) have solid evidence that hormone therapy reduces dementia risk we also don't have good evidence that the formulations we use today increase it.

Even in late-starters. ❎

Transdermal estradiol + micronized progesterone are my go-to hormones in late-starters. ✅

In some studies, early start hormone use is associated with lower dementia risk. 🎉

But, what we can't say is that taking hormones increases dementia risk.

Have you been told you can't take hormones? Why? ⁉️

Women live, on average, almost five years longer than men. 🎉

This is true across every continent, every socioeconomic group in every society that measures mortality.

Why don't we talk about this? ❓

Our XX chromosomes and ovaries provide the hardware.

The programs that run on top of the hardware are all hormone-sensitive - your responsive mitochondria, hypervigilant immune system, adaptive body, and connected brain.

So, yes, you're stacked (just not the way they meant). ✅

P.S. for purposes of this post, "female" means being born with ovaries.

Taking iron but not seeing improvements in ferritin or hemoglobin levels? 🤔

How you take your iron is as important as what type you take.

🔺Take it with Vitamin C.
🔺Take it away from coffee, tea or calcium.
🔺Take it every other day.

I like iron bisglycinate (fewer GI effects) but ferrous sulfate is the least expensive and also works well.

I wrote a Substack on low ferritin recently, which has more info.

But the hormone you pick at 70 matters more than it did at 50, and what you get depends on what you're chasing.

From ER to hormones and sexual medicine.

I'm a board-certified emergency physician with ten years in the ER before leaving in 2013 to learn about hormones, lifestyle medicine and (eventually) longevity medicine.

I got into sexual medicine because my patients would ask me, after I'd helped them with sleep, mood, and energy - "Now can you help me with my sex life?"

I feel so lucky to get to practice a form of medicine that includes focusing on all aspects of human health, including sexual health.

I found a new favorite laser! 💥

A week ago I got a micro-peel laser treatment from @biorestoration. This is a moderate depth, erbium ablative laser from @sciton_inc .

Within a week, I noticed improved skin texture and glow as well as less pigmentation.

I expect to see continued improvements in the coming weeks.

Does estrogen therapy cause melasma? 🤔

Estrogen + UV light can activate abnormal melanocytes to worsen melasma in some cases.

But, in clinical practice, we mostly see this with high dose oral estrogens, not lower doses of topicals. ✅

If you notice melasma while on estrogen therapy, make sure that you're a) also taking progesterone and b) always wearing UV protection! ⛱️

Yes, we should all listen to our bodies, but why is it that when it comes to anything where the data is thin, we default to "women are fragile"?

I wrote this week's free Substack for my twin daughters who graduate from high school on Friday. It's about cold plunging, yes, but it's about so much more. ❄️

It's about the way we talk to ourselves. About the way we talk to other women.

Women are not weak. We don't require kid gloves. And the sooner we stop telling ourselves we do, the sooner we can get on with changing the world.

Are you iron deficient? ❓

You don't have to be anemic to be low on iron. ❎

The lab's reference range for ferritin is 15-150 ng/mL but symptoms can start when ferritin is < 75-100.

If you have fatigue, hair shedding, restless legs or ice pica (cravings) get your Ferritin checked.

Your floss. Your mascara. Your tap water. 😲

In April 2024, the EPA set the first-ever enforceable federal limit on PFAS in drinking water. 🎉

This month, the Trump EPA proposed walking it back — pushing compliance to 2031 and rescinding limits on four PFAS entirely.

The science didn't get weaker. The protections did. 🔬

By the EPA's own math, the 2024 rule was projected to prevent 9,600+ deaths and 30,000 serious illnesses PER YEAR.

TLDR: They won't protect you. You have to protect yourself. ❎

Normal means "common" not "healthy" ❎

If we had a pool full of sick fish and studied them to figure out what was "normal" for those fish, we'd get lab value reference ranges that reflect what's normal for those fish. 🦈

NOT what's normal for healthy, thriving, happy fish.

Because the major labs consider the middle 95% of the population studies as being "normal" your lab values only tell you whether you're in that 95% or not.

My "optimal" ranges may differ slightly from other doctors but the idea is the same - you don't want to be a sick fish. 🐠

Can we delay menopause? 🤔

My new friend, Dr Kutluk Oktay (@innovation_fertility_pc ) says ... Yes!

Dr Oktay is an MD, phD - a fertility doctor and Yale ovarian researcher.

He is a pioneer in the field of ovarian tissue cryopreservation (OTC) as both a way to preserve fertility (for example in cancer patients) and delay menopause. 🎉

The idea is this: A small chunk of the ovary is taken out through a small laparoscopic incision. It's cryopreserved. Years later, it can be reimplanted and it will start working again!

Dr Oktay and I had the best time at @vitalistbay
(I'm pretty sure we became best friends) - we even had matching footwear!

A ferritin of 15 isn't normal. It's your body rationing iron. 🩸

Half the studies that built the "normal" range didn't even exclude iron-deficient women from the healthy group. Sick defined "normal."

Women have more testosterone in their bodies than estrogen.

Testosterone begins to decline around age 25 in both men and women. 😟

By midlife, many women notice symptoms of low testosterone, including low libido, mood changes, lack of motivation, and difficulty building muscle.

While testosterone is not FDA-approved for women, it's been used clinically in women for more than 50 years.

And not just for sexual purposes. ❎
