Does Hormone Therapy reduce heart disease risk? 🤔

Early-start oral estradiol with the right progestogen reduced CV disease risk in three clinical trials. 🎉

Transdermal (TD) estradiol is associated with lower CV disease risk in many observational studies but doesn't yet have a big, randomized controlled trial showing benefit.

Oral estrogen is better for lipids like LDL, ApoB, HDL and Lp(a). TD is better for triglycerides. ✅

So, yeah. It's complicated.

It's not too late! ❎

It's NEVER too late for low dose vaginal estrogen.

And, it's likely not too late for systemic estrogen (ex estradiol patch and oral progesterone).

Early start transdermal estradiol improved several markers of Alzheimer's disease, including blood, spinal fluid and PET scan markers. ✅

Oral conjugated equine estrogen (Premarin) was either neutral or harmful, especially in late-starters. ❎

Most of the brain research we have used older forms of hormones, not the body identical forms we use now.

This is reassuring data - hopefully the first of many good studies.

Your first period at 10 raised your breast cancer risk. Your estrogen patch at 52 doesn't.

Wait... what? 😲

The difference is timing. Early estrogen goes to work on young, undifferentiated breast cells while hormone therapy in midlife affects more "settled" tissue.

Early menarche is also often associated with metabolic risks, like obesity, as well as environmental chemicals, which can also increase breast cancer risk.

And the risk people love to pin on hormone therapy? It mostly traces back to old synthetic progestins (not micronized progesterone)

Do GLP-1s reduce the risk of breast cancer? 🤔

Possibly. But, with a few guardrails.

In the studies showing correlation with significant cancer reduction the women were overweight or obese. This was true for breast cancer and endometrial cancer. ✅

Other studies show correlation to lower colon cancer. ✅

But, we don't yet have studies showing lower cancer risk in normal weight, metabolically healthy people. Nobody has done those studies. ❎

And only observational studies this far show this relationship between GLP-1s and cancer. No RCTs.

Why are "peptides" being handed out like candy from bodega stalls in many cities?

Why are teenagers buying gray market peptides from the Internet?

Why have we convinced ourselves that peptides are both magically effective and magically safe? 🎱

BTW, GLP-1 medications and insulin are also peptides but they're FDA approved and backed by tons of human data. I'm talking about other peptides.

What's your experience been? ❓

Loss of hormones due to dwindling ovarian egg count is the reason for symptoms in peri-/menopause. 🤔

And, while a healthy lifestyle can certainly help, even the healthiest of women may have significant symptoms. 🥦

Hormone therapy not only relieves symptoms, it also helps prevent many of the future diseases associated with estrogen and progesterone loss.

Have you ever heard this common myth that "only unhealthy women need hormone therapy?" ❓

A new study linked GLP-1s to 30% less breast cancer across 110,000 women — but
it's observational, so it shows association, not proof. And the headline skips who it actually helps.

Extra weight? The signal is strongest.

Diabetic? Depends what else you're taking.

Healthy weight? We don't have the data yet.

It's a multiplier, not a magic wand. I broke down every study in this week's free

A few wild numbers...

20% of marriages in the U.S. are sexless.
17% of millennials check their phones DURING sex. 📲
40% of men over 40 have some degree of sexual dysfunction.
40% of women have sexual dysfunction.

And, about 75% of people with sexual dysfunction are currently untreated. 😲

Only 1.7% of women over 40 are taking hormone therapy. 😩

Only 3.5% of women in their 50's take hormones.

This Mayo Clinic study from 2026 looked at hormone prescriptions through 2023.

We have no problem prescribing other medications to treat many of the downstream effects of hormone loss but we won't prescribe the hormones that potentially fix the underlying problem. ❎

Hopefully, with the recent black box warning removal we'll see improvements.

You've been sold a half truth. 😲

Yes, muscle is the longevity organ. Yes, protein matters.

But sarcopenia, the slow strength-loss that quietly steals women's independence, doesn't start

Speaking and traveling are so fun. 🎉

I talked to a group of trail-blazing regenerative physicians in Vegas at @thestemcellconference this weekend.

The topic? How ovarian hormones affect not only female longevity and healthspan but our ability to heal and benefit from stem cell procedures once those hormones are gone.

I've been injecting stem cells since 2014. At that time, it was considered quite "fringe"

Luckily, medicine is changing. And I'm here for it!

What hasn't changed ... me, frantically huddling
in hallways and bathroom stalls, changing clothes on the fly and packing/repacking before making airport runs.

Ah, the glamour! 😂

Hormone therapy and dementia... 🤔

While we don't (yet) have solid evidence that hormone therapy reduces dementia risk we also don't have good evidence that the formulations we use today increase it.

Even in late-starters. ❎

Transdermal estradiol + micronized progesterone are my go-to hormones in late-starters. ✅

In some studies, early start hormone use is associated with lower dementia risk. 🎉

But, what we can't say is that taking hormones increases dementia risk.

Have you been told you can't take hormones? Why? ⁉️

Women live, on average, almost five years longer than men. 🎉

This is true across every continent, every socioeconomic group in every society that measures mortality.

Why don't we talk about this? ❓

Our XX chromosomes and ovaries provide the hardware.

The programs that run on top of the hardware are all hormone-sensitive - your responsive mitochondria, hypervigilant immune system, adaptive body, and connected brain.

So, yes, you're stacked (just not the way they meant). ✅

P.S. for purposes of this post, "female" means being born with ovaries.
